Symptom Tracker

PTSD UK Symptom Tracker

At PTSD UK, we know that feeling in control of your condition is important, especially when you’re waiting for treatment or actively going through therapy. Managing PTSD or Complex PTSD (C-PTSD) can feel overwhelming, but tracking your symptoms is a powerful way to regain a sense of control. Identifying patterns and triggers can provide valuable insight into your responses and symptoms and help guide your recovery.

That’s why we’ve developed the PTSD & C-PTSD Symptom Tracker, a practical tool to help you monitor and understand the symptoms you’re experiencing. This tracker is based on the diagnostic criteria from the World Health Organisation (ICD-11) and the American Psychiatric Association (DSM-5), and it’s designed to give you a structured way to log your symptoms and identify any patterns that emerge over time.

Important: This tracker is not a diagnostic tool for PTSD or C-PTSD. It is designed solely to help you observe, document, and understand your symptoms.

Key Features:

  • Daily Logs: Easily track your symptoms day by day or over a number of weeks or months with multiple sheets, providing a clear record of your experiences.
  • Identify Triggers: Helps you pinpoint triggers by tracking your symptoms over time.
  • Supports Conversations with Healthcare Providers: Use your tracker results to provide valuable insights during healthcare provider appointments or therapy/treatment sessions.
  • Print at Home or Order a Copy: Print the tracker yourself, or order it from our PTSD UK Supporters Store for just £1 per sheet (sheets can be photocopied for ongoing personal use).

Why Use It?

Whether you’ve already been diagnosed or believe you might have PTSD or C-PTSD, tracking your symptoms is a practical way to gain a deeper understanding of your condition. By regularly using this tracker, you can:

  • Gain clarity on the patterns of your symptoms.
  • Recognise triggers that contribute to symptoms.
  • Provide your healthcare provider with a detailed overview of your symptoms, which can be helpful during assessments or treatment discussions.
  • See your progression of reduction of symptoms throughout treatment.

Who Is It For?

  • Individuals awaiting treatment: If you’re waiting for therapy, tracking your symptoms can give you valuable insight into your condition while you wait.
  • Those in therapy or treatment: Use this tracker to monitor your progress and share your experiences with your therapist.
  • People who think they might have PTSD or C-PTSD: If you’re noticing symptoms but aren’t yet diagnosed, use the tracker over a few weeks and take it to your GP/mental health team to assist in discussions about your mental health.

Start taking control of your PTSD or C-PTSD journey today. Download the tracker for free to print at home or order a physical copy from our PTSD UK Supporters Store for £1 per sheet.

Please Note: This tool is not intended to replace professional diagnosis or treatment. Always consult a healthcare professional for formal assessments.

Treatments for PTSD

It is possible for PTSD to be successfully treated many years after the traumatic event occurred, which means it is never too late to seek help. For some, the first step may be watchful waiting, then exploring therapeutic options such as individual or group therapy – but the main treatment options in the UK are psychological treatments such as Eye Movement Desensitisation Reprogramming (EMDR) and Cognitive Behavioural Therapy (CBT).

Traumatic events can be very difficult to come to terms with, but confronting and understanding your feelings and seeking professional help is often the only way of effectively treating PTSD. You can find out more in the links below, or here.
