Safety Plan Worksheet

PTSD UK Safety Plan Worksheet

Living with Post Traumatic Stress Disorder (PTSD) or Complex PTSD (C-PTSD) can feel overwhelming, especially when you’re triggered or experiencing a flashback. In these moments, it’s easy to forget the coping strategies, contacts, or tools that you know can help you. That’s why we’ve created the PTSD & C-PTSD Safety Plan Worksheet—a practical, easy-to-use resource that empowers you to feel more grounded and supported.

This worksheet is available as a free download to print at home or as a professionally printed version that you can order online for just £2.50 per worksheet.

Why a Safety Plan Worksheet?

At PTSD UK, we know from personal experience how hard it can be to remember the things that have worked for you in the past when you’re feeling triggered or during a flashback. This worksheet acts as your personalised guide, created by you, for you, to support you when you need it most.

Having a safety plan ensures that all the strategies, reminders, and resources you rely on are readily available in one place. It’s a powerful tool to help you feel more prepared, reduce overwhelm, and take back control during challenging moments.

What’s Included?

The PTSD & C-PTSD Safety Plan Worksheet includes sections to help you:

1. Identify Your Warning Signs

Pinpoint the physical, emotional, and behavioural signs that indicate you’re feeling triggered.

2. List Trusted Contacts

Document the people you can call for support, including friends, family, or healthcare professionals.

3. Use Affirmations & Reminders

Write down calming statements, such as “This feeling will pass” or “I am safe now,” to ground yourself.

4. Note Your Safe Places

Identify physical locations or visualisations that help you feel secure.

5. Outline Steps to Take When Triggered or During a Flashback

Create an actionable plan for managing moments of distress, from grounding techniques to contacting your support network.

Why It Works

By preparing your safety plan during calm moments, you’ll have a reliable resource to turn to when your mind feels clouded or your body is in fight-or-flight mode.

It’s also a helpful tool to share with trusted people, like a family member, friend, or therapist, so they know how to support you effectively.

Treatments for PTSD

It is possible for PTSD to be successfully treated many years after the traumatic event occurred, which means it is never too late to seek help. For some, the first step may be watchful waiting, then exploring therapeutic options such as individual or group therapy – but the main treatment options in the UK are psychological treatments such as Eye Movement Desensitisation Reprogramming (EMDR) and Cognitive Behavioural Therapy (CBT).

Traumatic events can be very difficult to come to terms with, but confronting and understanding your feelings and seeking professional help is often the only way of effectively treating PTSD. You can find out more in the links below, or here.
